We’ll email required intake and consent forms to complete before care is scheduled.

In-Home Nursing Visit
A licensed nurse visits the patient’s home for medical assessment and monitoring. Physicians provide care and oversight virtually when appropriate.
Step 3
In-home visits are for medical evaluation and monitoring only and do not include caregiving or custodial services.
Short-Term Post-Hospital Care Plans
For patients recently discharged from the hospital or recovering after a procedure, TeleMD4U Urgent Care may recommend a short-term in-home care plan.
These plans typically span several days (often 7–10 days when appropriate) and are designed to support recovery at home and help reduce unnecessary hospital readmissions.
Care recommendations and pricing vary based on medical needs and are discussed during the initial phone call.
Individual in-home visits may also be scheduled without a multi-day plan.
