1. Choose the option that matches your situation.

• Please select one option below based on your type of condition and how soon you need care.    

            Urgent Care — Scheduled (24+ Hours) — $50

               For new, short-term problems that are not ongoing and can wait until tomorrow or later.

               Examples: sore throat, sinus infection, UTI, bronchitis, rash, minor illness.

           Urgent Care — Same Day (0–24 Hours) — $60

             For new, short-term problems that need attention today or by tomorrow.
             Examples: worsening sore throat, UTI symptoms today, fever, sudden illness.

           Chronic Care — Scheduled (24+ Hours) — $60

            For ongoing or long-term conditions, follow-ups, or routine management.
            Examples: ADHD, anxiety/depression, diabetes, asthma, thyroid care, weight loss, annual checkups.

          Chronic Care — Same Day (0–24 Hours) — $75

            For ongoing conditions that need attention today.
           Examples: medication issues, mental health worsening, asthma flare, urgent refills.

2. Complete your Telemedicine Consent Form.

After you book, you will receive a DocuSign email to sign your consent form.
This form must be completed before your virtual visit.

3. Join your virtual visit.

Your appointment confirmation will include a Google Meet link.
Click the link at your scheduled time to meet with the physician.
